Description of Duties:
Provide case management services to a caseload of 10 - 12 pregnant and parenting teen mothers between the ages o 17.5 - 20 years old.  Familiarity with wards of the state.  Strong working knowledge of DCFS, preparing service plans, case notes, case summaries and linking teens to community resources.  Attend juvenile court, administrative case reviews, weekly member staffings and communicate with external service providers.

Qualifications and Experience Required:
Bachelor's degree in Social Work or related field. 
Child Welfare License and CERAP required.
Must have car and drivers license with current 100,000/300,000 insurance coverage.
Must be able to pass a background check and fingerprinting.
Must have two years or more experience working with aggressive teenagers and DCFS.
Computer literate and good writing skills essential.
