Case Manager - Foster Care and Adoption Services

SUMMARY

 

The Case Manager - Foster Care and Adoption Services has primary responsibility for permanency planning for the children in his/her caseload. The Case Manager assesses the child and family and develops and implements a service plan in conjunction with available parents and other professionals and paraprofessionals. The Case Manager makes referrals for needed services. Based on assessment, service planning and provision of services, the Case Manager writes reports and makes recommendations to the Juvenile Court and the Department of Children and Family Services as required under contract and by policy and procedure.

 

Qualifications:

 

Ideal applicant must be licensed to practice child welfare in Illinois and certified in DCFS Rule 402 and Child Care Act.
Applicant must have 2-3 years experience and/or a Master's degree in Human Services field is desired.
Applicant must have a valid Illinois Driver's License and access to a car.
Applicant must be organized and able to deal with multiple/competing tasks.
Applicant must have good oral/written communication skills.
Bi-lingual English/Spanish is a plus.

Mental Health and Family Support Specialist

 

SUMMARY

The Mental Health and Family Support Specialist is responsible for the coordination and implementation of mental health services for children in the 0-5 program. The Mental Health and Family Support Specialist will complete psycho-social intake assessments and service plans, conduct group therapy with children, and provide expert consultation and support to parents, other caregivers, and teaching staff. The Mental Health and Family Support Specialist is also responsible for coordinating parental engagement and involvement in the program. The Mental Health and Family Support Specialist also assists with recruitment and enrollment for the Early Learning Program.

 

Qualifications:

·                     Master’s degree in Social Work, Psychology, Counseling, or related field

·                     Minimum three years experience working with young children and families in an early childhood field such as therapist, social worker or other related position; 5 years preferred.

·                     LCSW, LCPC or related license preferred or license eligible.

·                     Minimum of two years experience providing group therapy to children preferred.

·                     Bi-lingual Spanish preferred.

·                     Experience working with persons with chronic illness

·                     Head Start knowledge preferred.

·                     Excellent organizational skills.

·                     Excellent written and oral communication skills.
